Event Blog: Safer Leadership Learning Session 1


Beginning in April 2016, Salford partners; Salford Clinical Commissioning Group, Salford City Council, Salford Royal NHS Foundation Trust, Greater Manchester West Mental Health NHS Foundation Trust and Haelo, embarked on an ambitious improvement programme to make Salford the safest health and social care system in the UK. Over the next 2 years, health and care partners across Salford will build on learning from the successful Making Safety Visible programme.

Safer Leadership developed and working with partners aims to inspire leaders across Salford to think and act differently to bring about city wide change. Changing how we think, so we can change how we act.

Using the Measurement and Monitoring of Safety Framework, leaders will be challenged to reframe their perception of safety by assessing the health and care system through three lenses, Past, Present and Future. Changing how we think, so we can change how we act!

First up is Haelo Associate Director of Digital and Innovation, Abi Harrison, who opens Learning Session 1.

Next up is Francine Thorpe, Director of Quality and Innovation at Salford Clinical Commissioning Group and Executive Lead for Safer Salford who starts the Safer Leadership programme with a contextual presentation on the Journey Towards a Safer Salford.

Francine highlights the how Safer Salford is an evolution and continuation of the successful Making Safety Visible (MSV), commissioned by Haelo which ‘challenged thinking of board members and really started to talk about safety.’

When reflecting on the MSV, Francine describes the programme as ‘really positive to get shared learning across organisations’ and that it ‘challenged thinking about safety and safety improvement.’

Francine reflects on how the Measuring and Monitoring of Safety Framework can be adopted and implemented to help ‘shape the way we start to think differently about safety.’ Francine asks the question: how do we make the system safer for the people using our services? She believes that ‘safety improvement should underpin our integrated care system’ and that the health and care economy can learn from other industries.

In terms of culture, we need to ‘shift thinking’ – move on from past harm to include elements of present and future.

Francine closes by describing how this is an unique opportunity for leadership development.

Penny Martin, Chief Operating Officer at Haelo, gives an energetic and enthusiastic talk on changing the way we approach and view safety.

Sarah Garrett, Innovation Consultant, who worked on MSV now presents on the Framework, Concepts and Road Testing. This presentation gives background on the history of the Measuring and Monitoring of Safety Framework and how it came to be.

Sarah Garrett leaves the participants with the below quote and asks how we can ‘change the lens’ and start thinking differently about healthcare safety and harms.

Back from the first refreshment break, Penny Martin leads the first exercise of the day. Leaders are asked to mix disciplines and discuss where on the Maturity Matrix there part of the system fits.

Jane Carthey, one of the authors of Measuring and Monitoring of Safety Framework, created the Maturity Matrix to enable organisations to be openly transparent when discussing safety based on Manchester Patient Safety Framework (MaPSaF). Haelo and Safer Leadership Programme Manager Kayleigh Price introduces the Safety Measurement and Monitoring Matrix (SaMMM) and asks teams to be ‘open and honest on where your organisation / system sits in the five domains of the Measuring and Monitoring of Safety Framework.’ This will be revisited at a later stage in the programme.


We are back after lunch with Haelo Associate Director of Digital and Innovation Abi Harrison kicks off the afternoon with widening our view of past harm – what does harm mean to you? by asking leaders the following questions;

What does harm mean to you?
What do you think harm means from a patient/person perspective?
What measures of harm do we have?
Are we measuring the right things?

Abi encourages dialogue on tables discussing those key questions around harm which is now opened up a wider discussion to the whole room. After reflections, Abi moves the session on to What good looks like? Highlighting how the leaders in the room ‘know how you should and could use your data to its best.’ Choosing your measures is an integral element to safety of which there are 6 key elements:

Develop a strategy and align measurement to it
Check the balance of the dashboard against domains of quality
Use data form observation and report as well as numbers
Understand your data sources
Measure from the persons perspective
Measure across pathways and health economies

Next up is Nick John, Lead Data Analyst at Haelo, who discusses data over time, measurement for improvement and triangulating data.

Abi now introduces Carl Macrae, Expert in Reliability and Improvement Science Fellow, who is here to discuss achieving reliability in healthcare: learning from other industries.

Penny Martin now delivers closing comments, summarising todays insights and learning.
